In a lackluster encounter at Old Trafford the Foxes took the lead in the 42nd minute when Wilfred Ndidi, who was playing his first match in two months due to injury, had his shot inside the box blocked by Andre Onana only for the rebound to hit the head of De Cordova-Reid and sailed into the net.
But United rallied round in the second half and equalized through substitute Joshua Zirkzee in the 68th minute.
Harry Maguire nodded home Bruno Fernandes free-kick late into extra time to take the defending champions into the fifth round.
Harry Maguire became the first Manchester United player to score a winning goal in second half stoppage time in the FA Cup since Anthony Martial did so against Everton in April, 2016.
Some of the fourth round matches that will take place on Saturday will see Leyton Orient host Manchester City as Everton take on Bournemouth. Wigan will host Fulham while Brighton welcomes Chelsea to the American Express Stadium.
