City successfully defended their title with 98 points while Liverpool came second with 97 points.
Chelsea finished third with 72 points after they drew goalless with Leicester City.
Tottenham Hotspur finished fourth with 71 points after they drew 2-2 with Everton.
In other final day results, West Ham hammered Watford 1-4 while Southampton and Huddersfield settled their encounter 1-1.
Relegated Cardiff City turned the Theatre of Dreams into Theatre of Nightmares with a 0-2 victory over Manchester United. Newcastle United hammered relegated Fulham 0-4 while Crystal Palace defeated Bournemouth 5-3.
Arsenal were 1-3 better than Burnley.
